The standard offering on the BT TV boxes hasn't changed - though I would argue that it's never been fuzzy vision. It's standard SD (576i) on the standard channels, full HD (1080i) on the HD channels, and 4k (2160p I believe) on the Ultimate channel.

HD is still usually an extra cost, same as elaewhere as it doesn't appear to be fully standard in the UK

OK, how about you look at it another way:

If the SD option was removed and HD became standard, then the price would need to increase to cover the lost income. So instead of you needing to pay more for HD, everyone would need to pay more - even those who were happy with SD.

Yet TV subscribers only get fuzzy-vision unless paying extra, despite it not costing any more to provide the stream.

Secondly take 5G. BT/EE spent hundreds of millions of pounds for 5G licences that only covers certain areas of the country (I still have no 3G!). However, despite an actual huge extra cost to provide it, those benefiting from 5G pay not a penny more that I do for no data coverage whatsoever.
