Cannot seem to claim the BT rewards card

Hi,

I've been trying to claim the BT rewards card for a while now, but when I click on the account it doesn't show any info. However if I click on one of the old accounts it says no rewards.

Can someone have a look so I can claim it please?

Also can you remove the two old accounts that still show? Pm me for a screenshot of the numbers if needed.
