Cannot claim BT reward card

I recently signed up to BT Broadband through uSwitch. I received a link to claim my BT reward card but each time I click through on the link I'm led to a blank page. It's been about a month since my service has been activated. Please can someone help me with this? I don't want to miss out on my reward!
