Unable to redeem reward card

Seems like a common problem, I am trying to redeem my reward card for starting a new broadband service but am presented with a blank screen/strip when selecting the correct account.

Seems to be an issue with people who have an old/existing account by other posts on here.

Any help much appreciated.
