Find BT application form for Priority Repair Scheme

Where do I find this form to download on BT website? A helpful customer adviser told me I can find it online (if she posts me one, it cd take 4-5 weeks!) but I can't find it to download. Help plse!

Seems obvious that they don't reaally want people on the scheme and so are makng it almost impossible for Customers and Basic Level Staff to find this form.

IF you ever get one, then you will need it signed and stamped by a member of the General Medical Council, then returned to BT, then processed by them.

So all in all that will take so long that some potential users of the scheme wil have died by then!
