£50 Cashback cheque not arrived

Hi 

Signed up with BT on 22nd March  and it went live on 4th April.  I signed up and requested the £50 cheque on the link provided but nothing received. 

The £125 pre-loaded debit card has arrived (thanks!) but never received the cheque.

Can a Mod let me know how best to deal with this please?
