And another amazon voucher issue

Hi, I looked through all the posts on this issue and I have gone through all steps suggested. But no success. Here is my sequence of events.

 

22nd Oct - BT Mobile SIM Activation

22nd Oct - Voucher claimed via online form

5th Dec - No voucher received so sent a chaser via another online form (enquiry ref - 161205-001298)

13th Dec - Since I received no response to my 5th Dec chaser, sent another one (enquiry ref - 161213-003941)

18th Dec - Got a response from the Vouchers team that "reward is already being processed and you should receive this within the next 5 working days"

5th Jan - No voucher yet. Sent another email chaser

 

Can you confirm what I need to do to get the amazon voucher that I was promised when I moved my mobile (& phone + broadband) number to BT?
