Another Amazon voucher not received!

I got the mobile Sim only plan in Jan and haven't received the voucher, claimed a month ago and chased a few days ago but still not even a single acknowledgement..................

Sorry for the delay receving your Amazon voucher and thanks for confirming that you have completed both the claim and chase form.  Can you send me over your details so that I can check what's causing the delay sending out the voucher?
