Another Amazon voucher not received

I ordered 4 x BT Mobile plans 2nd October and activated all sims on the 5th October claimed Amazon voucher same time.
no vouchers have turned up as of today 05/12/16

filled contact bt form as supplied in previous posts on 24/11/16 email ref no: (*****-*****). still no vouchers or reply


Purchase date 02/10/16
Activated date 05/10/16
Claimed date 05/10/16

Filled enquiry form out 24/11/16

2 x 500MB plan, 2 x £20 Amazon voucher on 500MB plan, 
2 x 4GB plan 2 x £60 Amazon voucher on 4GB plan.

Sorry you’ve not received your gift card as yet.

I have manually raised your claim to dispatch to avoid further delay. This will be processed, sent for delivery and should arrive with you within the next 15-21 days minus any unforeseen issues.

But wasn't for a sainsburys gift card it was for 4 amazon gift vouchers?.
