Which BT Home Hub are you using?
I think you'd be more likely to get and answer from the Chromecast Community Forum.

The message means that for some reason, your chromecast and the device you are using to control it, are not connected to the same wireless network, so they appear to be isolated.
smart setup can also interfere, so disable it.
this is what it comes up with.
I am not really familiar with the home hubs, but if you have the original smart hub, its possible to separate the 2.4 and 5G wireless bands which gives the 5G one a separate SSID.
You can then make sure that the phone and the chromecast both connect to the 2.4GHz band.
Perhaps another forum member with a smart hub (not smart hub 2), can give you help.
I was having the same issue. Just turn off mobile data when setting up your chrome cast so that it doesnt connect to mobile network during setup when it briefly disconnects from wifi during setup.
