Hello,
I signed up for BT Infinity at a higher cost than available via other ISPs, but the promise of a reward card made it competitive.
I followed the procedure and claimed the card online once my Broadband had been activated.
The My Reward page shows "Dispatch Date - 26 Sep 2017" and to allow 20 days..
It has now been 30 days, and I have not received the card. Please can you look into this.

Hi @infinityuser Sorry you have not received you Reward Card yet.
Please Complete This Form to chase your Reward Card and the team will update you usually with 48hrs.

I have completed the form and now have an inquiry reference number.
I will wait another 48hrs and update the thread.
I have received the following response to the inquiry:
" Sorry you’ve not received your voucher as yet. I can confirm that voucher is on its way to you and you should receive this within the next few days. I’m sorry for any inconvenience caused.
"
Original Dispatch Date was 26 Sep 2017. I will wait a few more days as suggested, and keep this thread updated.

Thank you for sending in your details. I gave your mobile number a quick call, I'm sorry I didn't get speaking to you today. I'm sorry you haven't received your Rewards Card. I've raised this with the Rewards Team and I've asked them to investigated. I should receive an update within the next 48 hours and I'll be back in touch as soon as I know more.

I've received word back from the Rewards Team. They apologised for the delay and confirmed that the Rewards Card in on the way. It should arrive by the 30th of October. I'll follow up with you on the 1st of November to make sure.
If you receive the Rewards Card in the meantime, please post back.
