No, I have not heard anything from the voucher team yet. Not sure what their workload is like so do not know how long to wait before chasing them.

@salfeit Sorry about that, I can see notes indicating the voucher team have reissued you with your reward card on the 30/11/16 and it can take up to 30 working days to arrive.
