Cashback claim with BT

Hello,

Sorry to have to bring this up again (there seem to be quite a few posts about this topic)...

I joined BT Infinity Broadband, redirected to BT website from uSwitch. The deal included a cashcard and cashback. I've applied for the cashcard, but cannot find anything that allows me to claim the cashback. The BT live chat was not at all helpful. I'm fairly certain it is BT offering the cashback, not uSwitch.

Can anyone help? Is there a form I can fill in? Is there someone I need to email? Can a moderator help (as with other posts on this issue)?
