Claiming Reward Card

Hi there, anyone know how to claim the Reward Card? When I signed up, it said up to £90 Reward Card was available but I've had no notification about this and have now been with BT for more than 14 days. Hope I can claim - especially as signal dropped out completely momentarily tonight!

I'm sorry for the confusion claiming for your reward card.  Can you please follow the advice on the link that @Keith_Beddoe  posted above?

Tried the above but it says "Sorry we can't bring up your details at the moment please try again later"
