How do I claim the Amazon voucher and BT reward?

We signed up in June and we're activated at the beginning of July for BT WIFI.

When joining I was told I'd be sent an Amazon voucher after 30 days. We've heard nothing. When I go on the claim my reward page it says I have no rewards to claim. 

How do I check I definitely have a claim and claim for it?
