Best remedy I've found so far is an Android mobile, Google maintain a good list of spam numbers and you can see on your screen before you take the call if you should answer it or not.
Simply give people your mobile number instead of your landline number, reject any calls on your mobile google identify as spam and don't answer either phone to anyone who is not in your contacts list.
Any valid calls that are missed will be diverted to voicemail for you to retrieve at your leisure

They have an inbuilt scammer call list, and they have to announce their name before you take the call, this scares most of them away. If they do say their name you have to accept the call. Your friends / family can get straight through if you allow them enough times.

If a caller is announced then you don't have to accept, you can choose to block or send through to voicemail. Also friends and family will still have to announce who they are, regardless of how many times they have rung, unless you add their numbers to your phonebook
