I took the deal from MSE website in May and claimed the prepaid master card and cashback cheque by two different forms when my braodband up and running. The form asking What do you need to enquire about: Drop down menu not showing any option about forum enquiry so i chose email enquiry option. i hope that's right.

Don't worry what you choose in the drop down. As long as the body of the form explains what the issue that'll be fine!
