Disappointed to report that I have similar problem.
I have sent a follow up email today but feel deeply frustrated that the administration of this scheme is so flawed and promises are broken in such a cavalier fashion. It is clear from this forum thread that there are basic problems here that are simply not being addressed - other than by kind intervention of the mods from time to time.
Can a BT moderator please get in touch with me as well?

I have same issue as everyone else fibre optic package ordered in May installed June been chasing her reward card since .
was confirmed I was entitled and have been advised 3 times that I will have in next 10 days . Six weeks on and 4 emails latter still waiting , what a shambles
