The warning message has gone from the claim page and so I filled in my details. The Captcha is present and it took my details, leaving me with the Thank you for choosing BT page.
Not entirely fixed though, as it still insists on an 18 character order reference, but it's not compulsory so I left it blank.
Took 3 attempts for the Captcha as well, due to uncertain characters e.g. whether 0 or O.

Further to my post on 4 April, my Amazon voucher cards arrived today. They had been posted First Class Signed For, but the postman had just put them through the letterbox.
I was surprised to get them so soon - just 8 working days after claiming, especially given the grief some others have received.
