Returning a BT Hub

I accidentally ripped the postage label and I cannot read what it says on it anymore meaning I do not know where to post the BT Hub. I do not know where I can find the address and was hoping someone here can show me where i can find the postage address to return the BT Hub.

You can take it into a BT/EE shop or arrange a Parcelforce pickup, you don't need an address label
