Reward card

Hi folks hope someone can help i purchased the phone line and superfast unlimited broadband from bt.com it went live on the 14th of may but for some reason i cant see where i can claim reward i spoke to a chap on the online chat he gave me a link to a reward claim page i done this on thursday morning and i have still not heard anything.

Thanks for getting back to me robbie but for some reason i cant send you a message i put all the details in but my email comes up as yellow and the submit your question box is faded out strange
