Rewards Card and 2 day delay on activation

Hi, 

Can someone please help with trying to locate and obtain my rewards card? I've logged in from links so many times, it's ridiculous. However, i keep being drawn to the same home page with no mention of a rewards.

Also, my activation was 2 days late which caused issues with me working from home.

Have your services been active for 14 days yet and how much was the reward card for?
