I used this link https://bt.custhelp.com/app/contact_email/c/6090 to try to claim suport in claiming my reward card - see below.
The form wont go through and I keep getting a message saying the 'Date in wrong format please re-enter using format DD / MM /YY' I put the date as 20/01/18 When was your BT Mobile Sim Activated.
I am trying to claim my reward card for the Sim only contract I ordered in Dec. I used this link https://www.bt.com/s/apps/appsclaim/index.html?view=myrewards#/accounts/23377662/rewards and it shows I have already claimed the other reward for the Broadband and TV contract but not for the mobile phone. I realise I have to wait 30 days from the point I activate the sim card but i t is well passed that date now. Could you help me this please?

Yes it takes me to a log in page, then after I log it it takes me to the same I linked to in my opening post.
I click on Claim Rewards amd it says No rewards available at the moment.
What next?

I was so happy last time I posted and now I dont know what BT are playing at. The Reward Card has not arrived.
When I signed up for the deal I took into account the amount the Reward card was going to give me and that made it the best deal avaliable at the time. If the cashback fails to arrive I feel that BT has misold me the mobile contract and I would like to know how to cancel it so I can return to EE or O2.

BT could just sort the Reward Card business out. Can anyone tell me when I will receive the cashback of £100?
Some people might be thinking that BT just expect a percentage of customers to forget to check the offer has been fulfilled or not bother following it up. I dont blame Neil its the system BT set up.

As @NeilO is looking into this he will contact you directly. But what normally happens is that he will contact the rewards team to chase it up and pass on any feedback. Rewards are dispatched by a third party company and most of the time it works as it should but some times it falls short. Normally its upto 30 days after a moderator chases it up. You won’t get it the next day I’m afraid, but once a moderator helps they normally keep an eye on things..

I have been unable to access my original BT Community account and so created this second one.
I clicked on you name and another page loaded but I cant see a way to contact you, or to report the problem. Has BT changed the way the Community deals with customer requests ?
Could you give me new instructions please?
