I first placed an order for my BT Services (phone, broadband & TV) on 14th May 2015. My services went live on 29th May that year.
I'm just wondering when the 12-month equipment changes I would have to pay if I left my contract early, expire? The charges would be for my BT Homehub 5 & BT YouView Box. Would it be this year on the on 14th May date I first applied for my BT services, or the 29th May date my services got activated?
You'd need to speak with someone who works within BT for help with that. Otherwise it's just speculation about the charges. Get in touch with the chat team about it today and they'll give you an estimate of everything.

I've now talked to BT support via Live Chat and they have given me the correct date for when my equipment charges expire - the correct date falls closer to the date my services were activated, rather than when I first signed up.
