if you are still in test socket with filter and there is no noise on quiet line test then you need to phone 151 and get engineer visit again

My suggestion would be to get back in touch with BT and explain what's happened since that engineer visit. You've tried a replacement router, inside the test socket and by the sounds of it, the engineer had a look at your internal setup and wasn't able to find any issues there. He then made some changes within the external network which temporarily improved those intermittent problems. If your speed slowed after that visit, it sounds like the line was reset in an attempt to stabilise the connection. If it's still dropping, let BT know so that they can escalate this.
