After some really good advice from @NeilO and @licquorice which I didn't have time to act on my problem was resolved by raising a complaint with BT. Within 24 hours of submitting a complaint a broadband engineer was despatched to sort out the problem at the exchange and then he turned up (unnanounced, but still welcome) at the house to check that I was happy wth the new speed.

 

My experience suggests that it's worth trying the usual route and persisting with it, but when you feel like nothing's happening and you're still stuck with the fault, then raise a complaint instead. BT seem to react to that very well.
