Amazon £100 Voucher not sent

I have the same issue.  I bought two BT Mobile phones in September 2017, one £100 Amazon voucher was sent and now that I am chasing BT for the second voucher, they are telling me it wasn't automatic, but that I needed to chase them . . . this seems very unfair, as well as not a good business practice.  Here is the response from BT.

You can only claim your BT Reward Card once your mobile is installed - you have 3 months from the order date to claim. After 3 months your claim will expire and you will no longer be able to claim.

I am sorry we will not be able to claim the card for you.

How can I get help on this?  Who do I contact since customer service won't help?
