I have exactly the same issue. When I complained a sales person tried to sell me a BT Sport package and said the Black Friday offer was no longer available to me. Also did not see it on the order but the £100 reward card wasn't listed there either so I wasn't too worried...plus my activation date was cancelled band I was left with no internet for a week so had other things to worry about. I really feel badly treated by this. BT should (in my optinion) not be trying to trip me up. Can I pleas have the BT Sports offer finalised.
