BT Reward Card not on my 'Claim Your Reward' account

I've recently purchased a BT Fibre 2 Broadband package (activated on the 12/08/20) which included a BT reward card (MasterCard) as part of the deal. However, it is past the cooling off period and it isn't appearing on the 'Claim a reward' page. I can’t find a way to contact BT regarding this. Can anyone help please? TIA
