Can’t claim virtual reward card

I can’t claim the virtual reward card despite lots of reminders sent by BT. I am presented with a list of accounts (3 old accounts) to choose the account with the reward and the radio button for the new account cannot be selected. This is more than frustrating given it’s £160!

Im having the same issue, been getting emails telling me to claim it and all I get is
