When our 18 month contract comes to an end, do we have to do anything if we are quite happy to continue with the same deal we have and just carry on but out of contract.

You can of course, but the price will revert to the standard rates for your package, which will cost you more.
If you wish to negotiate another reduced cost deal, you would have to ring the BT Value Team on 0800 800 030.
What @Keith_Beddoe has said is correct. Once the 18 month term is over your service will continue to work as normal. Any discount will stop at the end of the contact and you'll be charged at the standard price.
