I can certainly do that for you.
Before I send you a private message, can you try and log into your MyBT at https://www.bt.com/mybt.
Do you have an option showing to return your kit to BT?
This is the advice we give on https://www.bt.com/returns:
Returning your kit
When we need you to return your kit, you'll see an option to 'Return your BT kit' after logging into My BT.
Follow the steps provided and we'll send you a white returns bag in the post within 7 days.
If we've asked you to return equipment and you need another bag or need one sending to a different address, you can order one easily by you can order one easily by logging into My BT and selecting 'Return equipment' under 'Return your BT kit' 7 days after your original request.
If you try to order one before this time your order will not be processed.

I have swapped over to EE, got whole new set of equipment from EE and have a BT hub and disks to return, but no request to do so. There is no way of ordering a bag, so what do I do?
you can order a returns bag by phoning CS 03301234150 and requesting one
