Reward card not arrived

I have contacted BT again about my missing reward card and for the second time have been fobbed off. I have had my activation email on the 2nd February with instructions that my card will arrive in 7-10 days, and yet the card still has not arrived.

Please can someone help as the BT customer service man said I need to fill out a missing reward claim form which can be found when I log onto myBT, and searching for it. After an hour I have still not had any joy...

I have seen others with similar issues which have been resolved, so hoping mine will be too
