Reward card

Took out broadband and a mobile sim. I have managed to claim for my £80 reward card for the broadband through claim my reward but it didn't come up with the £45 reward for the mobile and I can't find out how to claim for the mobile sim.
The end date for the broadband claim was 26th June.

What date was your mobile SIM activated, as you have to wait 30 days after SIM  activation before you can claim?

My SIM was activated back in March, so past the 30 days. Was expecting an email about it but got nothing.
