Trying to complete a home move online

Hi all,

 

Trying to move my BT Infinity to my new address online and when I try it I keep getting a technical error has occured.

 

It is finding my new address etc but I can't get further than the moving date screen.

 

Would a mod please help me achieve this home move, was also hoping for a much better deal as I have been out of contract for some time and have been paying much more than I normally do.
