Rewards card - Home Broadband

Hi, I ordered broadband and currently waiting for it to go live. I am eligible for a reward card and have had a few emails telling me that I can go online NOW and order it. 

However when I logged on with my BTID it gave me a message saying im not due any rewards and I haven't claimed any. 

Please could you advise how I can go about placing an order for this reward card?
