Do I have to have a BT landline and Broadband account to be able to have BT Premium mail?

BT Premium mail is for non BT Broadband customers. There is nothing premium about it, it is just the exact same product as standard email that BT broadband customers have but paid for.

Maybe a stupid question but why would you want to pay for BT Premium mail instead of just creating a GMAIL or any other free mail service which is just as good or in my opinion better
Not sure what you are wanting to do.
BT Premium Mail is for customers who already have a btinternet email address and wish to keep it after moving away from BT broadband. You can't just apply for Premium mail . Why on earth would you want to pay £7.50/m for a rubbish ISP mail service when you could have free email address from Gmail or Outlook anyway?

You are on a 30 day rolling contract so you have to give 30 days notice.

I would also question why you would want to pay £7.50 per month when gmail is in my opinion far better and is free.
Why?
I am just leaving a BT landline and broadband account. The broadband speed is partcularly poor (1mbps) with no plans for any improvement in the next few years, if at all, and line rental at over £20 is expensie and not realistic.
In order to have time to work out getting a Gmail account - it is not easy to undertsand - and change all my current email addresses I need something in the interim to keep in touch with the outside world.
That is rural living!
Why didn't you say that in the first place. Your original question seemed as though you weren't a BT customer.
If you don't want to pay £7.50/m for premium mail, you can just keep your address free as a Basic Mail account. This is only available as webmail, you cannot use mail clients or phone apps.
