You will have to use a DDNS service that is supported by the BT home hub. No-IP is one such service. You can assign a host name which will always point to your public IP address.
You create an account with them, and then enter the details into your home hub.

My understanding is that unless you are paying big money for a Static IP address you will have to set up your router to use a Dynamic DNS service. On your smart hub see Home -> Advanced settings -> Broadband -> Dynamic DNS. Of course I could be completely wrong (for which I apologise) as I have never had the need to use it.
