So I have a second, smaller box in my bedroom. The aerial is plugged into the box , the network cable is plugged into the box and the extender connection plug in the wall plug, and the HDMI cable comes from the TV to the box. But I cannot use the apps on the TV. So does this mean I need a splitter to take the aerial to my TV also?

No. You need an internet connection to the TV as well. A simple network switch should suffice to split the signal from extender connection plug to get a network connection to both the box and the TV.
