It would only be available if you have a police warrant due to criminal activity and the records are required as evidence.

A GDPR request isn't a police warrant.

No, as far as I'm aware the only way to obtain the information is the police with a warrant. You certainly won't be able to get it for personal reasons.
ISP do not have your browsing history. Nearly every web server uses HTTPS to encrypt its connection to your browser.
When accessing an encrypted site the only data the ISP sees is the IP address of the server and the quantity of bits go over the connection. They don’t see the page you accessed, they don’t see search queries or any other form data.
