I went live with my broadband on 14 September. I've been expecting to be able to claim the reward card in line with the offer provided at sign up but cannot see how to do this. Can you help, please?

If your order qualifies (placed online during an eligible time) then you can claim via https://www.bt.com/manage/bt-reward-card/
Cards need claimed within 3 months, activated within 3 months of receiving and the balance spent within 12 months.
