On Openreach side they also may tie this to a NAD Key and that then gets used for network routing mapping.

Sounds like an Address Matching Issue as opposed to a, being connected to the wrong Exchange Issue.

As mentioned, the length of Fibre can be far greater than a Metallic Path.

Longest Fibre I’ve worked on is 35km from the Head End, so just shy of 22 miles. 

I’ve heard of some Fibres reaching over 50km from the Head End in North Wales. How true that is I don’t know.

An update:

I've spoken to BT/EE again and went ahead with the recommended "cease and reprovide", taking some extra care to help with the address issue. I was on FFTC, which could explain why it's not working if they think my address is in another town. 

I've now been upgraded to FTTP, or at least I will be on September 8th, so maybe I'll be back online then. Some of you mentioned that with FTTP it wouldn't really matter if I was connected to a town 10-15 miles away, so hopefully it will just work.

I also went on the Royal Mail postcode finder and reported the issues with my address, because that really does seem to be the root of a lot of my issues (even my mini hub got sent to wrong address in the other town today by DPD) I am still waiting to hear back from that.
