Please someone help me, I am a disabled person...

Please someone help me, I am a disabled person and I am a BT subscriber, but when I switched to BT broadband internet I did not know about this help with reduced bills for disabled people. Please guide me on how I can get this disability help.
I appreciate any help you can provide.
