Not sure I follow, OP asks if having DV is an obstacle to moving to a broadband only provider, the answer is is No it doesn’t, so the BT USO is moot , if a customer doesn’t want telephony that’s their choice, if you are suggesting they want BT telephony only and broadband from another source , then currently BT have an obligation to provide it , but there is no differentiation between DV or PSTN in that respect, how a separate telephone service could be delivered if the customer had broadband only over the Openreach connection presumably would be a problem to resolve should it ever occur.
Oh, last night's classic on Sky's forum was entitled "B" and consisted of "Broadband superfast upgrade" and that was it.

As far as I know, there always has been a charge for the phone. At least, I've been charged separately. £3 is quite good. Last time I looked the new customer price was now £5.
Having telephony with BT ( irrespective of being on Digital Voice or PSTN ) doesn’t present a problem to the new provider of broadband and telephone if they want to import the number from BT to themselves they can , it’s irrelevant which system the number is on provided the new provider requests the phone number in the appropriate way then that request is fulfilled , obviously the new provider needs to offer telephony , not all do , but that’s hardly BT’s problem if a competitor only offers broadband.
