Thanks for the update, Openreach should have completed the full commissioning tests, and this issue would have been found.
Most likely a faulty port within the cabinet, or an internal cabinet wiring issue. These do take longer to be resolved, as they have to be looked at by a specialist team.
I think you have just been unlucky, as most installs are without problems.
