We are getting poor quality iPhone calls when at home making/ receiving mobile calls whilst connected to the router/ mesh system with our iPhones. The issue is voice dropouts. When disabling wifi, the calls have no voice dropouts. The calls stay connected but we miss words in the conversations. Are there any known issues? causes? of this. VOIP calls with the BT phone are not affected with this issue.

To me, this sounds like interference with Wi-Fi calling or poor mesh network connectivity. You should try updating your router or mesh network firmware.
Be sure to make sure Wi-Fi calling is enabled on your iPhones, and if possible, assign a separate SSID for the phones or adjust the range settings to reduce congestion.
