BT added 12 months onto my mobile contract

I took out a 12-month mobile contract last year which ends  16/09/22.  However, when I try to request the PAC code it adds another 12 months onto it until 17/09/23 and says I have to pay an early termination charge of  £200+ .  This is clearly wrong as I am sure I took out a 12-month contract and that is what it says on my plan summary too. 

I have attached 2 screenshots 
- one shows the right contract end date in 2022 

- one shows the wrong contract end date in 2023

You’re going to need to contact BT CS and get them to help you. You can call 150 from your mobile or 0800 800 150 (Option 3, 6) from any other line.
