Equipment Returns Bag

I've had a new BT Pro Box and have been emailed twice and now sent a text asking that the old equipment be returned in the bag provided. I wasn't sent one and I can't see within the My BT app how to request one. No  Customer Service on Chat and no one picking up at the phone. Any suggestions?
