I have 3rd party access / nominee status on my Mum's BT account. She is paying around £90 per month for a telephone and broadband package (Fibre Halo 3 and Complete Wi-Fi) which is a much higher standard of broadband speed and reliability than she needs - she hardly ever uses the internet and certainly does not need a high reliability or high speed service and this should have been obvious from her usage statistics to whoever sold / upsold her this contract. How can I find out when this contract was entered into and what checks were run at the time as to the suitability of this contract for her needs? I believe for example as part of this contract she was provided with a wi-fi disc which she never even set up because (i) she didn't need it (ii) she didn't know how or what it was.

if you can login to MYBT you should find the details of the last contract and start and expiry date
there are no suitability checks it is up to customer to decide what to order and whether package extras are what they need
