If you call BT 0330.1234.150 you can request that they send one out to you.
Make sure you keep a copy of your Proof of Posting and if possible take a photograph of it in case you somehow lose the paper copy.

When I recently cancelled one of my two BT accounts it was just a label they sent so I rustled up my own box and packaging for the return.
